  • Overview

    Partner with service recipients and their families, in developing self-determination, and problem solving skills, while building circles of support and connections within the community, to foster inclusion for service participants. Work as an active member of an individual's support team, to assess safety needs, identify goals, and develop effective staffing strategies; and serve as a liaison between individuals, service coordinators, family support coordinators, and community support professionals. 

    Responsibilities

    • Manage a small caseload, maintaining staffing according to the individual's needs
    • Provide direct support services to aid the development of individuals within their home and community
    • Provide individual support in areas of budgeting and personal finance, medical appointments, and home health management, and monitor housing arrangements

    Qualifications

    • Bachelor's degree or combination of High School diploma and relevant experience
    • Effective interpersonal and organization skills
    • Must enjoy working in a collaborative team environment
    • Ability to meet the agency's approved driver criteria
